Dark circles are one of the most common concerns I hear about at Boutiq, and also one of the most frequently mistreated. The reason? There is not one type of dark circle. There are at least three distinct causes, and each responds to a different treatment. Applying the wrong treatment to the wrong type of dark circle produces disappointing results, which explains why so many people have tried multiple products and procedures without success.
Understanding which type of dark circle you have is the most important diagnostic step, and it is something I assess at every under-eye consultation at Boutiq Medical Clinic in Orlando.

The Three Types of Dark Circles
Type 1: Pigmentation (Periorbital Hyperpigmentation)
This is the most common type in darker skin tones but occurs across all skin types. The under-eye skin appears brown, gray, or purple due to excess melanin deposition in the dermis. Contributing factors include:
- Sun exposure and UV-induced melanin production in the thin periorbital skin
- Post-inflammatory hyperpigmentation from repeated rubbing, contact dermatitis, or eczema
- Genetic predisposition, particularly in individuals of South Asian, Middle Eastern, Mediterranean, and East Asian descent
- Hormonal changes that affect melanin regulation
Type 2: Vascular (Blue or Purple Discoloration)
The periorbital skin is the thinnest skin on the body, typically less than 1mm thick. When the underlying blood vessels, particularly the orbicularis oculi muscle's dense vascular network, are visible through this thin skin, a blue or purple discoloration appears. This type is more prominent in people with naturally fair or thin skin, and worsens with:
- Fatigue and sleep deprivation, which cause vasodilation and increased blood pooling in the periorbital area
- Allergies, which cause repeated rubbing and capillary damage
- Natural aging, as collagen loss progressively thins the overlying skin and makes the vasculature more visible
Type 3: Structural (Hollow Tear Trough)
The tear trough is the groove that runs from the inner corner of the eye diagonally down toward the cheek. In youth, a smooth transition between the lower eyelid and cheek makes this area appear full and even. With age, fat compartment loss, descent, and skin laxity create a hollow depression that casts a shadow, producing the appearance of dark circles even when there is no pigmentation or vascular component.
This type increases with age and is often accompanied by puffiness or under-eye bags, as the remaining orbital fat compartments become relatively more prominent compared to the surrounding deflated areas.

How to Tell Which Type You Have
There is a simple clinical test: gently stretch the skin under your eye. If the darkness moves with the skin and lightens or becomes more brown in color when stretched, it is primarily pigmentation. If it lightens significantly and reveals that the color was partly due to the underlying vasculature, the vascular component is significant. If the darkness appears more as a shadow that is created by the hollow geometry of the tear trough region, the structural component dominates.
Most patients have a combination of two or three types, which is why a clinical assessment at Boutiq is valuable before beginning treatment. Treating one type while the others remain unaddressed limits the overall improvement.

Treatment by Dark Circle Type
For Pigmentation: Light-Based Treatment
Pigmentation-type dark circles respond to treatments that target excess melanin in the periorbital skin. The two primary options at Boutiq are:

Vascular dark circles are visible because the skin is too thin to hide the underlying blood vessels. Thickening the overlying dermis reduces their visibility. Treatments that stimulate collagen production in the periorbital skin produce meaningful improvement over time:

What Does Not Work and Why

They improve surface hydration and may have mild benefits for very superficial concerns, but they do not address the structural, vascular, or deeper pigmentation causes of significant dark circles
- Concealers and color correctors: valuable for temporary camouflage but not a treatment

Why do I have dark circles even when I sleep enough?
Sleep affects only the vascular component of dark circles, and only temporarily. Dark circles caused by melanin pigmentation, volume loss in the tear trough, or naturally thin periorbital skin are not resolved by sleep. If your dark circles persist despite adequate rest, the cause is structural, pigmentation-based, or related to skin quality, all of which require clinical treatment rather than lifestyle change.
Can BBL Hero treat dark circles?
